Start with the payer, then drill into the strongest CDT-level policy evidence. Pages below are generated from Axlow's validated policy corpus, not a generic list of insurers.
Axlow intentionally keeps this directory selective. A payer profile must contain meaningful validated rule depth, multiple policy records, and enough substantive evidence to support useful code-level research. CDT pages use an even stricter threshold: at least five substantive validated rules across at least five policy records and two geographic scopes.
